About the role

A remote clinician role within a collaborative care programme that embeds mental health support in primary care. The clinician assesses patients, builds treatment plans and coordinates their mental and physical care.

What you will do

Create treatment plans, encourage patient engagement and arrange follow-up care
Deliver brief evidence-based interventions such as behavioral activation and motivational interviewing
Support medication management by tracking adherence, side effects and treatment response
Monitor patients by video, phone or in person and record encounters in the EHR
Refer patients to psychosocial treatments and adjust plans when progress stalls

What they look for

A master's degree in social work, psychology, counseling, rehabilitation or a related field
Three to five or more years of clinical behavioral health work, case management or care coordination
Experience working with various patient populations
Ability to meet monthly performance metrics set by clinical leadership
