What you will do
- Oversee scheduled submission of care management report and outreach files to contracted health plans
- Collect, check and merge data from several internal systems for accuracy
- Confirm members stay eligible for programmes and coordinate reauthorisation through provider portals
- Keep records consistent across platforms by copying and tagging case notes, then auditing for gaps
- Compile weekly status reports and trend summaries for executives and department leads
- Review and update member profiles, discharge reasons and service end dates
