Nurse Practitioner, Kidney Care (Hybrid)

About the role

Deliver integrated care to people living with complicated kidney disease, combining remote telehealth work with visits across an assigned region. The aim is to cut hospital stays and raise the quality of life for patients.

What you will do

• Carry out full health assessments that cover medical, social, emotional and financial needs
• Manage chronic and end-stage kidney disease, working to slow decline and avoid readmission
• Use prescribing authority, medication oversight and test interpretation alongside kidney specialists
• Guide the clinical team so care matches treatment plans and addresses psychosocial needs
• Travel to dialysis centres and kidney practices within the assigned area

What they look for

• At least two years of practice as a nurse practitioner
• An active, unrestricted nurse practitioner licence for the state of practice
• A DEA licence, or the ability to obtain one within 90 days
• Current CPR or BLS certification
• A valid driver's licence and car insurance in the state
• Intermediate skills with common office software and a home office with fast internet

Nice to have

• Five or more years as a family, primary care, internal medicine, cardiology, kidney or endocrine nurse practitioner
• Experience running population health work aimed at fewer hospital admissions
• Prior use of Cerner or a comparable electronic health record
