You support participants in a nursing home transition and diversion waiver program, coordinating care and service plans. The role suits someone with direct waiver experience who can handle several cases independently.
What you will do
- Coordinate care and services for waiver participants
- Create, review and revise service plans to match participant needs
- Liaise with participants, relatives, caregivers and providers to keep care continuous
- Track service delivery, follow up on gaps and resolve coordination problems
- Keep accurate case files and documentation meeting program rules
- Carry an independent caseload with prompt communication and follow up

- Familiarity with waiver regulations, service planning and care coordination
