Bilingual Clinical Nurse Coach (RN)

About the role

Provide remote health coaching in English and Spanish to members at risk of or living with chronic kidney disease, helping them manage their conditions and avoid dialysis.

What you will do

• Reach active members through video, phone, text and email in both languages
• Gather clinical information and build individualized care plans following NCQA guidelines
• Assess members' health status and care coordination needs
• Coordinate with members' health care providers when needed
• Use motivational interviewing and the Triple Aim framework to help members reach health goals
• Explain program benefits to new or inactive members and document information under HIPAA

What they look for

• Active RN license in good standing, with willingness to obtain licensure in other states
• At least five years of RN experience
• Availability for two late shifts and three day shifts weekly
• Native-level fluency in spoken and written English and Spanish
• Comfort working fully remotely and using computers and Microsoft applications
• Willingness to travel occasionally, including overnight stays

Nice to have

• Experience building care plans for chronic conditions or chronic kidney disease
• CDE or CDCES certification
• Prior fully remote nursing experience
