Case Manager Registered Nurse

About the role

Suits a licensed RN who likes coordinating care across clinical and social needs. The schedule is predictable and the work happens by phone, with rare in-person visits.

What you will do

• Assess members by phone, with occasional in-person visits
• Write and carry out care plans tailored to each person
• Spot clinical, social and behavioral needs and address them
• Work with a multidisciplinary team on coordinated care
• Help members understand benefits, resources and moves between care settings
• Use clinical tools and data to lower risk and improve outcomes

What they look for

• Active unrestricted multi-state RN license in your state of residence
• Three or more years of clinical nursing experience
• At least a year working across several systems such as Excel, Outlook and web tools
• Strong assessment, communication and critical thinking skills
• Willingness to get extra state licenses, paid by the company

Nice to have

• Case management experience, ideally in an integrated model
• Bilingual Spanish and English
