Clinical Nurse Coach (RN)

About the role

Provide remote health coaching to members who have or are at risk of chronic kidney disease, helping them understand and manage the conditions and behaviors that affect kidney health.

What you will do

• Reach and stay in touch with assigned active members through video, phone, text and email in English
• Gather clinical details and build individualized care plans following NCQA guidelines
• Evaluate members' health status and coordinate the care they need
• Work with members' healthcare providers when necessary
• Use motivational interviewing and the Triple Aim framework to help members meet health goals
• Explain program benefits to new or inactive members and document information under HIPAA

What they look for

• Active RN license in good standing, with willingness to become licensed in other states
• At least five years of RN experience
• Availability for PST hours with flexibility for evenings and weekends
• Strong computer skills, including Microsoft products
• Excellent written and verbal communication
• Self-directed, comfortable with ambiguity and able to handle competing deadlines

Nice to have

• Experience creating care plans for chronic conditions or chronic kidney disease
• CDE or CDCES certification
• Prior fully remote nursing experience
