About the role

A remote clinical pharmacist position supporting value-based cardiology programs. The role centers on optimizing guideline-directed medical therapy for high-risk cardiac patients alongside nurses and providers.

What you will do

Review cardiac medication profiles and assess therapy against ACC/AHA guidelines and payer quality programs
Spot therapy gaps, titration opportunities, contraindications, interactions and adherence barriers
Conduct telephonic medication reviews and educate patients in plain language
Partner with care managers and nurse navigators on medication plans and interdisciplinary case reviews
Document recommendations and interactions promptly in the care management platform or EMR

What they look for

PharmD degree
Active unrestricted Texas pharmacist license in good standing
Confident communicator with cardiologists and advanced practice providers
Patient-centered, empathetic and clinically accurate style

Benefits

Full-time W-2 position with benefits
40 hours per week, Monday to Friday
Business-hours overlap with Central Time
