About the role

Deliver primary care to a panel of patients with varied needs, blending in-clinic visits with occasional virtual care. The role combines acute, chronic and wellness care within a collaborative clinical team.

What you will do

• Manage your own patient panel covering acute, chronic and preventive visits
• See patients in the clinic or testing centers and handle some telehealth appointments
• Join weekly clinical rounds and keep building your skills
• Coordinate daily with on-site colleagues and virtual clinical teams
• Use your clinical training and take part in in-office procedures
• Obtain extra state licenses and credentialing for virtual primary care

What they look for

• Graduated from an accredited FNP or PA program and hold national certification
• At least two years of outpatient primary care with patients of all ages in the last five years, or one year plus a primary care fellowship o
• Hold a California state license by the start date
• Show commitment to human-centered primary care
• Communicate well and care for people from every background
• Use technology effectively to deliver quality care
