Remote Hospice Triage RN PT 4:30a-10a + rotating Sat & Sun 7:30a-4p CST

About the role

Provide after-hours phone support to hospice patients and their families, assessing needs and coordinating timely care as an extension of client care teams.

What you will do

• Handle incoming calls from hospice patients and families, assessing situations and deciding next steps
• Work with on-call teams to arrange care and connect people with resources
• Record every call and action accurately in electronic medical records
• Stay calm and professional while managing urgent or stressful calls
• Support clients' nursing staff during their non-core hours

What they look for

• Active multistate Registered Nurse license
• Comfort using technology and electronic medical records for call documentation
• Ability to type notes quickly in a busy setting
• Strong communication and critical thinking skills
• Capable of working independently from a remote location
• Reliable internet access and consistent attendance during all scheduled shifts

Nice to have

• Background in hospice, palliative or end-of-life care
