Remote Hospice Triage RN PT OVN 3 week rotation 10:30p-5a CST

About the role

You would give after-hours phone support to hospice patients and their families, assessing needs and coordinating timely care. The service extends clients' own care teams outside core hours.

What you will do

• Take incoming calls from hospice patients and families and assess their situation
• Decide on the right next step for each caller and help arrange care
• Work with on-call teams to line up resources and coordinate support
• Record every call and action accurately in the electronic record
• Stay calm and professional while handling urgent or stressful calls

What they look for

• Active multistate registered nurse license
• Comfort using technology and documenting in electronic medical records
• Ability to type notes quickly while handling a busy call flow
• Strong communication and critical thinking skills
• Able to work independently from a remote setting
• Reliable internet and consistent attendance during scheduled shifts

Nice to have

• Background in hospice, palliative or end-of-life care
