Remote Hospice Triage RN PT 3:30p-11p + rotating Sat & Sun 9:30a-4p CST

About the role

You will give after-hours phone support to hospice patients and their families, assessing needs and coordinating care. The role extends the client care teams during hours when their own staff are off duty.

What you will do

• Take incoming calls from hospice patients and relatives and assess their situation
• Decide on suitable next steps and arrange timely help
• Work with on-call staff to line up care and resources
• Record every conversation and action accurately in the electronic record
• Stay calm and professional when calls are urgent

What they look for

• An active multistate registered nurse licence
• Comfort using technology and electronic medical records for call notes
• Ability to type notes quickly while handling a busy call load
• Strong communication and critical thinking under pressure
• Capacity to work alone from home with steady attendance and reliable internet
