Remote Hospice Triage RN PT 3:30p-9p + rotating Sat & Sun 9:30a-4p CST

About the role

You will give after-hours phone support to hospice patients and their families, assessing needs and coordinating care. The role extends the client care team outside core hours.

What you will do

• Take telephone triage calls from hospice patients and family members
• Judge each patient's condition and decide the right next step
• Work with on-call teams to arrange care and resources
• Record every call and action accurately in the electronic record
• Stay calm and professional when calls are urgent

What they look for

• An active multistate registered nurse license
• Comfort using technology and electronic medical records for call notes
• Ability to type documentation quickly in a busy setting
• Strong communication and critical thinking under pressure
• Ability to work independently from a remote location
• Reliable internet and consistent attendance during all scheduled shifts

Nice to have

• Experience in hospice, palliative or end-of-life care
