Care Coordinator, MA (Bilingual)

About the role

Supports proactive coordination of care for people with long term conditions such as kidney disease, high blood pressure, diabetes and heart disease, working under clinical supervision to keep patients engaged and informed.

What you will do

• Help enroll and onboard patients into the chronic care program and confirm their eligibility
• Check consent paperwork is finished before any patient contact begins
• Reach out weekly by text message and phone to keep patients engaged
• Reinforce care plans set by providers during each patient conversation
• Track engagement time to meet billing requirements and flag patients who stop responding

• Comfortable working on the front lines of healthcare with a compassionate, patient focused approach
• Strong organizational skills and the ability to handle a large patient panel
• Clear patient communication skills and ease with high volume outreach
• Able to work 8:00am to 5:00pm Pacific time while fully remote
• Bilingual in English and Spanish

• Prior experience supporting nephrology or chronic kidney disease patients
