Field Care Manager, Behavioral Health

About the role

Owning integrated behavioral health care management for members with serious mental illness, substance use issues or serious emotional disturbance, including children and justice-involved people. The work pairs community visits with provider coordination and interdisciplinary team leadership.

At a glance

What you will do

• Meet members face to face in their communities and by phone to build trust and coordinate care
• Coordinate behavioral and medical services and keep providers engaged with treatment plans
• Lead the interdisciplinary care team, run its meetings and plan care transitions
• Help members understand their health and address social needs that affect outcomes
• Work with internal departments, providers and community groups to link members to services

What they look for

• Reside in Illinois
• Hold an active Illinois LCSW, LMFT or LCPC license, with no provisional or supervised status
• At least 2 years of post-degree clinical work in behavioral health
• Case management experience with complex SMI, SUD or SED populations
• Willingness to travel locally to homes and facilities for assessments
• Comfort using electronic medical records and other software

Benefits

• A $5,000 hiring bonus, half after 180 days and half after one year
• Up to $5,000 a year toward tuition and related fees

A typical week

• Face-to-face member assessments in homes and regional facilities
• Telephonic check-ins and care coordination calls
• Interdisciplinary team meetings with providers and care extenders
• Documentation in the electronic medical record
• Follow-up on care transitions and referrals to community services
