Care Coordinator

About the role

Support patients through their care journey by monitoring engagement, removing barriers and coordinating with providers and facilities when extra help or higher levels of care are needed.

What you will do

• Track patient paperwork and requirements so people are ready for a first appointment
• Follow up on outstanding items and spot barriers to timely activation
• Activate patient accounts and prepare them for care using set workflows
• Explain next steps, appointments and coordination processes clearly to patients
• Answer patient questions and concerns and ensure proper follow-through
• Coordinate with providers and facilities when patients need more support or a care transition
