What you will do
- Make outbound calls to people referred by health plans, providers, hospitals and community groups
- Explain available services and help eligible individuals enroll
- Assist with Medi-Cal and benefits applications where appropriate
- Coordinate warm handoffs, including for justice-involved individuals
- Connect members with lead care managers for ongoing coordination
- Represent the organization at community outreach events
