Care Manager

About the role

Assess and coordinate care for members with mental health and substance use needs, creating personalized plans and educating members and families. The role is remote within Florida and follows a weekday daytime schedule.

What you will do

• Evaluate member needs by phone or home visits and recommend or arrange the right care plan
• Build and update care plans for members with high acuity, finding providers and community resources
• Coordinate among members, families, caregivers, community services and care providers
• Track member status and outcomes, noting treatment side effects or complications and adjusting plans
• Document member information and care activities to meet state, federal and payer rules
• Educate members and families on treatment options, referrals, procedures and benefits

What they look for

• Two to four years of related professional experience
• An active Florida license such as LCSW, LMSW, LMFT, LMHC, LPC or RN with behavioral health experience
• One year of professional pediatric care experience

Nice to have

• Medicaid experience involving foster care or adoption
• At least one year of pediatric experience
