Enhanced Care Management Program Manager

About the role

Lead an enhanced care management program so vulnerable populations receive coordinated, person-centered support. The role guides regional teams, oversees quality and budgets, and works to narrow health equity gaps.

What you will do

• Supervise lead care managers across counties through one-on-ones, team meetings and regular in-person visits
• Set benchmarks, quality assurance protocols and standardized workflows, using data to spot trends
• Oversee member services, including in-person visits, documentation and strategies to raise engagement
• Act as liaison with hospitals, providers and community organizations to improve care transitions
• Develop and manage program budgets, allocating staff and resources to match member needs
• Pilot new initiatives such as telehealth and prepare quarterly performance reports

What they look for

• Bachelor's or master's degree in healthcare administration, public health, nursing or a related field
• Five or more years in healthcare management, with three or more in care or case management
• Knowledge of care management principles, delivery systems, and Medicaid or Medicare program requirements
• Strong analytical skills and experience with data-driven program evaluation
• Leadership, communication and the ability to juggle priorities in a fast-paced setting
